In most cases when there is medical necessity of treatment (not cosmetic) we will file your insurance electronically.
You should verify your own coverage before scheduling treatment. Any amounts assigned to deductibles or co-pays are
due immediately. All amounts are due in full 45 days from the date of service whether your insurance has paid or not.
State law requires each claim to be paid or denied within 45 days. If insurance pays after appeal we will refund any
credit balance, however, appeals can take many months. Financing is available.
Deposit
A deposit may be required to reserve time in the schedule for a consultation or treatment. A credit card number
may be given instead of the usual $100 deposit when scheduling the initial consult. It will not be charged if you
keep your appointment or cancel with 48 hours notice. If you "No Show" or cancel at the last minute, your credit card
will be charged a $100 "No Show Fee" When you keep your appointment these charges will be billed to your insurance
company and not to your credit card. When scheduling vein closure procedures, a $100 deposit will be taken if no credit
card is on file. Failure to appear for a scheduled procedure or cancelling with less than 48 hours notice will result
in forfeiture of the deposit. It will be applied to the final treatment or refunded if no balance remains.
We have a long list of patients who would gladly move up if you cancel your appointment with at least 48 hours notice.
When you cancel at the last minute or don't show up, there is not time to fill your spot.
Bottom Line: This costs you nothing if you show up or cancel with proper notice. You do
this all the time when you make a hotel reservation.

It is your responsibility to check with your insurance carrier before treatment to verify coverage. There are literally
thousands of different insurance carriers and each carrier has many different plans. It is impossible for
our billing staff to know the specifics of each individual policy. We can tell you in general whether a particular insurance
carrier usually covers a procedure, but we can not tell you if they will pay in your specific case. You are fully responsible
for all charges not paid by insurance within 45 days. Cigna, Humana, US Healthcare, Anthem, United Healthcare, Medicare, Blue Cross/Blue Shield of Michigan (BCBSM), and Aetna have all announced coverage for EndoVenous Laser Ablation. It is important for each patient to verify their own coverage
because not all policies issued by each insurance carrier have the same coverage. For example some policies require
pre-certification and many require a trial period of wearing compression stockings before any other treatment will
be covered.
